What is gymnema?
Gymnema sylvestre is a woody vine native to India and Africa. Its leaves have been used in traditional Ayurvedic practice for centuries.
The Hindi name, gurmar, is often translated as sugar destroyer, because chewing the leaf is said to blunt the taste of sweetness for a short time. That folk note is where its reputation for sweet cravings and glucose comes from. It is sold as leaf powder, extracts and capsules, and the strength varies a great deal between products.
On the SugarReset label it appears as one name in the 22-name blend. The sales page features it as one of six.

Think of 25 mg as a very small amount next to a spoonful of dried leaf. The 25 mg ceiling is arithmetic and holds only if the names are listed in true order of weight, which is the normal rule for a proprietary blend. It is a limit, not an estimate of what your bottle holds.

What do trials of gymnema show?
Two recent reviews report lower fasting glucose in adults with type 2 diabetes. Both warn about weak methods.
A 2021 review pooled 10 studies with 419 participants and compared groups with their own starting values, not mainly with placebo. It reported lower fasting glucose, post-meal glucose and A1C after supplementation (Devangan et al. 2021). A 2023 review of six randomized trials found lower triglycerides, cholesterol and fasting glucose, and warned about heterogeneity, low study quality and a lack of dose information (Zamani et al. 2023).
A before-and-after comparison cannot separate the effect of a product from the effect of being watched, changes in diet and chance. That is why both sets of authors call for larger, better trials. The NIH, in its page on diabetes and supplements, adds that studies of supplements differ in people, formulations and length, so there are no clear conclusions (NCCIH, Diabetes and Dietary Supplements).

What are the cautions with gymnema?
The main caution is with diabetes medicines. If gymnema has any effect on glucose, it could add to a medicine that already lowers it.
That is the reason to ask a prescriber or pharmacist before use if you take insulin, a sulfonylurea or another diabetes medicine. Low glucose can cause shakiness, sweating, hunger, a fast heartbeat and confusion, and severe cases need emergency help (NIDDK, Low Blood Glucose). Keep checking your glucose as your care team told you.
Tell the surgical team about gymnema and every other supplement. Supplements that may change glucose are a particular concern before and after an operation.
Pregnant and nursing mothers, children under 18 and people with a known medical condition should consult a physician, as the label says. Stomach upset has been reported in some trials. If you have liver disease, ask a clinician first.

How do gymnema products differ?
Gymnema products vary in the part of the plant used, the extraction method and the share of the marker compound, gymnemic acid.
A raw leaf powder, a water extract and an alcohol extract with a set gymnemic acid content are three different products. Trials that report improvements used particular extracts, and the 2023 review noted a lack of dose information in some reports (Zamani et al. 2023). A label that says only “gymnema” cannot tell you which of these is in the bottle, and the SugarReset label does not say.
Quality is also a concern for supplements in general. FDA does not approve supplements before they are sold and does not routinely test their contents (FDA, Q&A on Dietary Supplements). We found no trial that tested gymnema inside a 22-name blend for cravings, so we cannot say.
The taste effect of the leaf is real but short, and it is about the mouth, not about hunger. A craving has many causes: sleep, stress, meals that are too small, habit and mood. A person who feels new, intense thirst or hunger with frequent urination should see a clinician, because those are signs that glucose may be high (NIDDK, Type 2 Diabetes).
